Access Control Application Process

The issued access/identification card is the property of the University of Santa Barbara and must be returned upon demand. Use of this card is for business purposes only.

 

  1. Click on the Access Control Application Button to download the Form.

  2. Please print it, and fill in the requested employee information.

  3. Sign and date the completed Access Card Application and take it to your Department Supervisor or Chair for authorization.
